Yes. Your membership includes a physical therapy assessment to determine your fitness level and recommend programs at the gym. We will provide you with a copy of our assessment along with the medical release form to give to your primary care physician for review and signature.

Most insurance providers offer gym membership benefits. Please reach out to your individual plan provider for more information on coverage. If needed, we can provide a Superbill if you want to submit for insurance reimbursement. In some cases, a prescription from your primary care physician for a gym membership will be needed in order to qualify for reimbursement.

This is not a bill. It is a receipt you can send to your insurance company to ask for reimbursement.
